When Immunotherapy is Considered

  • Recurrent pregnancy loss (RPL) with no clear cause
  • Elevated natural killer (NK) cell activity (in select research contexts)

It's not for everyone — it's most useful when standard investigations are normal, but immune imbalance is suspected.

Common Types of Immunotherapy

Corticosteroids (Prednisolone)

Suppress overactive immune responses to reduce inflammation and help embryo implantation.

Intralipid Infusions

Fat-based intravenous solution believed to help reduce NK cell activity and regulate immune function.

IVIG (Intravenous Immunoglobulin)

Contains antibodies from donor plasma to modulate immune response and improve maternal-embryo tolerance.

Heparin and Aspirin

Improve blood flow and may help with clotting-related issues that can affect implantation.

Paternal Lymphocyte Immunization (LIT)

Experimental treatment involving injection of partner's white blood cells to improve immune tolerance (rarely used).

Goal of These Treatments: To calm the immune system's overreaction and support embryo implantation.

Current Evidence and Guidelines

According to the latest ASRM and ESHRE guidelines:

  • Routine immunotherapy is not recommended for all patients.
  • IVIG and intralipid may help select patients with recurrent losses or RIF after other causes are excluded.
  • NK cell testing is not routinely recommended (testing lacks standardization).
